Volleyball Membership Waiver & Release of Liability

Please read carefully before participating in any volleyball activities.

By purchasing a Volleyball Membership, I acknowledge and agree to the following terms:

1. Assumption of Risk

I understand that participation in volleyball activities involves physical exertion and inherent risks, including but not limited to injuries such as sprains, fractures, concussions, or other serious harm. I voluntarily assume all risks associated with my participation.

2. Medical Fitness

I certify that I am physically fit to participate and have not been advised otherwise by a qualified medical professional.

3. Release of Liability

I hereby release and hold harmless [Amihan Surf International, LLC], its owners, staff, sponsors, and affiliates from any and all liability, claims, demands, or causes of action arising from participation in any volleyball-related activities.

4. Media Consent

I consent to the use of my image, name, or likeness in promotional materials, including photos and videos, unless I notify the organization in writing.

5. Emergency Contact

I agree to provide accurate emergency contact information if required and authorize event organizers to seek emergency medical treatment on my behalf if needed.

6. Agreement

By checking the box or signing this form, I acknowledge that I have read, understood, and voluntarily agree to this waiver and release of liability.
