Volleyball Membership Waiver & Release of Liability

Please read carefully before participating in any volleyball activities.

By purchasing a Volleyball Membership, I acknowledge and agree to the following terms:

Membership: All players are required to have a valid Amihan Surf Membership. 

·       General membership is $5.00 valid for 365 days and can be completed online. 

·      To obtain, renew, or check your membership click on "Profile Icon" located at the top of every page. Then select the drop down menu on the right hand corner with your username. Select profile from the drop down menu. Select "Manage Membership.

Volleyball Membership Waiver & Release of Liability

By purchasing this registration, I acknowledge and agree to the following terms:

1. Assumption of Risk

I understand that participation in volleyball activities involves physical exertion and inherent risks, including but not limited to injuries such as sprains, fractures, concussions, or other serious harm. I voluntarily assume all risks associated with my participation, including travel to and from the event, and accept full responsibility for an injury or damages that may occur.

2. Medical Fitness

I certify that I am in good health and physically fit to participate and have not been advised otherwise by a qualified medical professional. I authorize event organizers to secure emergency medical treatment on my behalf in the case of injury or illness.

3. Release of Liability

I hereby release, waive, discharge, and covenant not to sue Amihan Surf International, LLC, event organizers, volunteers, sponsors, and the facility owners for any liability, claims, demands, or causes of action arising from or related to any loss, damage, or injury that may be sustained while participating in this event or any volleyball related activities.

4. Media Consent

I consent to the use of my image, name, or likeness in promotional materials, including photos and videos, unless I notify the organization in writing. 

I understand that:

• I will not receive any compensation for the use of these images or videos.
• My name may be used in connection with the images or videos.
• All media becomes the property of Amihan Surf International, LLC.

5. Emergency Contact

I agree to provide accurate emergency contact information if required and authorize event organizers to seek emergency medical treatment on my behalf if needed.

6. Agreement

By purchasing this registration, I acknowledge that I have read, understood, and voluntarily agree to this waiver and release of liability.
